Findings
The cardiac silhouette is enlarged and rounded, with normal pulmonary vasculature. There is mild retraction of the lung lobes with increased pleural opacity. The thorax is hypoinflated on all images with an increased bronchointerstitial pattern. Within the abdomen, there is poor peritoneal detail.
DDx
- Cardiomegaly with rounded shape is most consistent with pericardial effusion. Cardiomyopathy is less likely.
- Pleural and peritoneal effusion secondary to right heart failure.
- Bronchointerstitial pattern may be due to hypoinflation or inflammatory or degenerative disease.
Diagnosis
- Severe pericardial effusion with cardiac tamponade diagnosed on echocardiography
- Right atrial mass suspected to be hemangiosarcoma
