History

5 month old female Siamese cat obtained from a shelter and has always been small and thin.

Decreased activity over the last month with sudden onset of tetraparesis.

Findings

Images shown are axial - T1, T2, FLAIR, T1 post contrast. Sagittal - T1, T2, T1 post contrast.

There is moderate dilation of the ventricular system. There is a pocket of fluid accumulation at the cerebellomedullary cistern. This fluid is slightly hypointense to CSF on the T2 weighted images. This fluid pocket is compressing the spinal cord ventrally.

There is marked meningeal enhancement ventrally along the brain stem. There is also rim enhancement of the fluid pocket at the cerebellomedullary cistern.

DDx

Abscess at the cerebello-medullary cistern with regional meningitis, spinal cord compression and obstructive hydrocephalus.

Diagnosis

CBC showed inflammatory changes (WBC 23030, PMN 19115, LC 3224), and the CSF had moderate, mixed inflammation (RBC 29, nucleated cells 91). CSF was submitted for FCoV PCR (neg) and CSF and serum were submitted for FCoV antibody titers (positive). These findings are compatible with a diagnosis of CNS FIP.
